Six years ago, my husband Scott and I found ourselves on the other side of this journey.
Scott’s dad, Fred Pierce, was rushed to a hospital here in Savannah. At the time, Scott and I were newly engaged. He was living and working in Atlanta, and I was here in Savannah.
When Fred was discharged, Scott dropped everything to come home and help. I still remember the moment of panic when Scott looked at me and said,
“Who is going to manage all of this? I live in Atlanta.”
And in that moment, everything became clear.
Even with my nursing background, there was no one guiding us through what came next. No one overseeing medications. No one coordinating appointments. No one helping us make sense of the many moving pieces that suddenly mattered so much.
When Scott had to return to Atlanta, the gaps became even more obvious.
At the same time, in my work as a hospice nurse, I was seeing this every day. Families were trying to coordinate care, manage complex medical needs, and support aging loved ones without expert guidance. They were doing their best — but the system left them to figure it out alone.
That experience changed everything for us.
We realized there had to be a better way. A model of care that didn’t ask families to become the system. A model where medical insight, daily support, and clear leadership worked together — not in silos.
That realization became the beginning of Coastal Care Partners.
We built Coastal Care Partners to fill the gap we experienced ourselves — to provide nurse-led, compassionate, personalized support for families navigating aging care.
Today, our work is built around three interconnected forms of support, designed to work together:
What makes this model different isn’t any single service.
It’s how the pieces are integrated.
When care is connected, fewer things fall through the cracks. Families feel less stress. Decisions feel clearer. And aging care feels steadier.
I share this not to tell you about a company — but to be transparent about where this philosophy comes from.
This blueprint was shaped by lived experience. By sitting in the same seat many of you are sitting in now. And by years of walking alongside families who needed someone to help them hold the whole picture.
As you continue reading, know this perspective is grounded in both professional experience and personal reality.
And you are not alone in this journey.
Many families come to us feeling overwhelmed, unsure of what questions to ask or where to turn next. If this story feels familiar, you’re in the right place.
When a parent’s health begins to change, families are often left trying to make sense of medical information, safety concerns, and difficult decisions — all at once.
Family Guidance Sessions are one-on-one virtual meetings with Amy, designed to help you understand what’s happening, clarify next steps, and feel more confident about the decisions in front of you.
These sessions provide virtual care management without the commitment of long-term services. They are focused, practical, and grounded in real-world aging care experience.
During a Family Guidance Session, Amy will help you:
These sessions are designed to reduce stress, bring clarity, and help families move forward with confidence.
